Spring 2025 SLO Reporting Form

DUE BY TUESDAY MAY 20 @ NOON

Before you complete this form, you should have the following:

• A complete list of the course-sections that you taught and the results from the individual assignments you used to assess each Student Learning Outcome for each course-section

• Access to information in self-service about your class roster

• Access to the Master Course Syllabus for your course. Current MCSs can be found here.

• Note: Some Master Course Syllabi that use our old forms may not have clear student learning outcomes. In those cases, please contact your chair for help identifying appropriate student learning outcomes.
• Note: Only include information about students who "audited" your class if they were assessed in the same manner as other students.
• Note: You may have some students who did not participate in every assignment that maps to an SLO. For more guidance on how to handle that reporting please see the FAQ here.
